JASKO Enterprises, Inc. (“JASKO”) may obtain your Motor Vehicle Record (“MVR”) — your driving record — from the licensing agency of each state in which you hold or have held a driver’s license, and/or from a consumer reporting agency acting on JASKO’s behalf, in connection with your application for employment and, if you are employed, periodically throughout your employment.
An MVR may include your license class, status, endorsements, and restrictions; traffic violations and convictions; reportable accidents; and suspensions, revocations, or other licensing actions.
Federal regulations require JASKO to inquire into the driving record of each prospective driver for the preceding three years (49 CFR 391.23) and to review the driving record of each employed driver at least once every twelve months (49 CFR 391.25).
Where an MVR is obtained through a consumer reporting agency, it is a “consumer report” under the Fair Credit Reporting Act (FCRA), 15 U.S.C. § 1681 et seq. JASKO will obtain such reports only with your written authorization, solely for employment purposes, and in accordance with the FCRA and the Driver’s Privacy Protection Act, 18 U.S.C. § 2721 et seq.
YOUR RIGHTS: You have the right to know whether a consumer report was used in an employment decision. If JASKO intends to take adverse action based in whole or in part on a consumer report, you will first be provided a copy of the report and a summary of your rights under the FCRA, and a reasonable opportunity to respond, before any final adverse action is taken. You have the right to dispute incomplete or inaccurate information directly with the consumer reporting agency that furnished the report.
By signing below, you authorize JASKO Enterprises, Inc. and its authorized agents to obtain your Motor Vehicle Record from the applicable state licensing agencies and/or consumer reporting agencies, for employment purposes, during the application process and, if you are employed, periodically throughout your employment as described above.
You understand that this authorization remains in effect during your employment unless you revoke it in writing, and that a revocation does not affect reports already obtained. You also understand that a photocopy, scan, or electronic copy of this authorization is as valid as the original.
Fill in your name, sign, and date above first. In the print dialog, choose “Save as PDF” to keep a copy with your entries, or print it to sign by hand.
