- ☐ Equipment ownership registration and inspection verified
- ☐ Required insurance evidence accepted
- ☐ Each proposed driver has a separate complete qualification file
- ☐ Clearinghouse full query completed for each driver
- ☐ Negative DOT pre employment drug test verified for each driver
- ☐ Random testing pool enrollment confirmed for each driver
- ☐ Road test or permitted equivalent complete
- ☐ Motive and required systems active
- ☐ Orientation complete
- ☐ Property receipt complete
- ☐ Trailer and escrow requirements complete when applicable
- ☐ No unresolved safety maintenance insurance or agreement hold
