This authorization permits JASKO to obtain and review motor vehicle records for employment and driver-qualification purposes, subject to applicable law.

AUTHORIZATION

I authorize JASKO Enterprises, Inc. and its authorized agents to obtain motor vehicle records from each jurisdiction in which I held a motor vehicle operator license or permit, for evaluating my qualifications, continued eligibility and compliance. This authorization does not waive rights provided by applicable law.

CERTIFICATION AND SIGNATURE

I certify that the information I provided is true, complete and accurate. I understand that an electronic signature has the same effect as a handwritten signature where permitted by law.
