Use this form to start or change payroll direct deposit. Submit it through JASKO’s secure payroll process. Do not send completed banking information through ordinary unencrypted email or driver chat.

EMPLOYEE AUTHORIZATION

I authorize JASKO Enterprises, Inc. to deposit payroll amounts into the account or accounts listed above. If JASKO or its payroll provider makes an erroneous deposit, I authorize a correcting debit only to the extent permitted by law and not exceeding the erroneous credit. I understand that I remain responsible for verifying the routing and account information and that processing may require one or more payroll cycles.

This authorization remains in effect until JASKO processes my written cancellation or replacement authorization, or until employment ends and all payroll obligations are completed. Cancellation does not affect transactions already initiated. I will promptly notify Payroll of account closure or changes.
