This form gives JASKO general consent to conduct limited queries of the FMCSA Commercial Driver’s License Drug and Alcohol Clearinghouse for a contractor driver seeking or maintaining authorization to operate under JASKO’s motor-carrier authority. It does not create employment and does not replace the driver’s specific electronic consent required in the Clearinghouse for a full query.
I consent to JASKO Enterprises, Inc., or its properly designated consortium or third-party administrator acting for JASKO, conducting limited queries of the FMCSA Drug and Alcohol Clearinghouse to determine whether drug or alcohol program violation information about me exists in the Clearinghouse.
This general consent authorizes multiple limited queries while I am an approved or proposed contractor driver under JASKO’s motor-carrier authority, beginning on the date I sign and ending when that authorization ends, unless I revoke this consent earlier in writing. JASKO may conduct limited queries as required by law and when otherwise permitted for its safety-sensitive driver program.
A limited query does not disclose detailed violation information. If a limited query indicates that information exists, JASKO must obtain my specific electronic consent in the Clearinghouse before FMCSA releases the detailed record. Each full query, including a pre-activation full query, requires specific electronic consent in the Clearinghouse. This JASKO form is not consent for a full query.
I understand that if I refuse or revoke consent for a required query, or do not provide required electronic consent for a full query, JASKO cannot permit me to perform safety-sensitive functions under JASKO’s authority, including operating a commercial motor vehicle, unless and until JASKO may lawfully do so.
