This form gives JASKO general consent to conduct limited queries of the FMCSA Commercial Driver’s License Drug and Alcohol Clearinghouse. It does not replace the driver’s specific electronic consent required in the Clearinghouse for a full query, including every pre-employment full query.
I consent to JASKO Enterprises, Inc., or its properly designated consortium or third-party administrator acting for JASKO, conducting limited queries of the FMCSA Drug and Alcohol Clearinghouse to determine whether drug or alcohol program violation information about me exists in the Clearinghouse.
This general consent authorizes multiple limited queries during my employment with JASKO, beginning on the date I sign and ending when my employment with JASKO ends, unless I revoke this consent earlier in writing. JASKO may conduct limited queries as required by law and when otherwise permitted for its safety-sensitive driver program.
A limited query does not disclose detailed violation information. If a limited query indicates that information exists, JASKO must obtain my specific electronic consent in the Clearinghouse before FMCSA releases the detailed record. Each full query requires specific electronic consent in the Clearinghouse. This paper or electronic JASKO form is not consent for a full query.
I understand that if I refuse or revoke consent for a required query, or do not provide required electronic consent for a full query, JASKO cannot permit me to perform safety-sensitive functions, including operating a commercial motor vehicle, unless and until JASKO may lawfully do so.
